1. Introduction
  Thank you for being willing to take this survey. This survey is part of a study looking at characteristics of faith-based poverty abatement and social justice ministries. There are other surveys that are going to clergy associated with faith-based agencies, and to guests who use such services. In no case will I be mentioning or naming specific agencies or persons, or publishing data that could identify specific agencies or persons.
This survey assumes that your agency is an explicitly faith-based agency -- that is, it is affiliated with a religious organization and/or was established because of religious beliefs. If yours is not an agency of this type, this survey is not appropriate for you.
There are about 60 questions in this survey; I anticipate that it will take you approximately 30 minutes to complete. If you need to stop in the middle of the survey, you will be able to come back and start from where you left off (or go back to the beginning to change your answers, should you wish to). Once you have completed the survey, however, you will not be able to open it again; the survey can be filled out only once by any individual.
If you would like more information about my project, and/or you are willing to provide input for later stages of this project, you will have a chance to indicate that at the end of this survey. The contact information you provide will not be linked to your answers.
