Re: Research questionnaire on RFID technology adoption in companies

My name is Al-Amin Abba Dabo, a PhD student attached to the Institute of Logistics and Operations Management, University of Central Lancashire, Preston. I am undertaking a research project to investigate the relationship between the adoption and implementation of Radio Frequency Identification (RFID) technology and business performance and competitiveness, in organisations around the world.

The project investigates links between RFID, business performance and competitiveness, and supply chain management. I would very much appreciate your contribution to this important research by completing this questionnaire. It will take only a short time (approx. 15 minutes) to complete this questionnaire as most of the questions require only a tick (). It will be most helpful if you could be as accurate as possible and return your responses within a week.

Questions 1 to 12 should be completed by all respondents.

Questions 13 to 28 should be completed IF your company has adopted RFID whilst questions 29 to 36 are to be completed by companies YET to adopt RFID.

Information for the study and the results will be used for academic purposes only; you and your organisation’s names will not be divulged, as strict confidentiality is assured. Additionally, personal data will be processed fairly and lawfully with due regard for individuals’ privacy and personal data security. To ensure data protection, files which contain personal data will be stored in locked cabinets, and electronic data in encrypted devices. Access to such data will be via secure, remote networks and will be limited to me alone (I am fully aware of their responsibilities under the Data Protection Act). In the event where participants wish to withdraw from the investigation, this can be communicated to myself and any data originally collected about them will be deleted/destroyed.
