  Statement of Disclosure

Dear Survey participant, my name is Jonathan Collins. I am currently conducting research in support of my Masters of Strategic Leadership. The title of my action research project is “Traveling Fast and Cheap: A Hostel Life”. My research explores the effects on training and employee satisfaction in regards to private ownership. As part of my research, I ask that you complete and return this survey. Your participation is voluntary and a decision on your part not to participate will not be held against you. Any information you provide will be confidential and retained in a secure location for a period of no less than three years. At the end of three years, all survey documentation and data will be destroyed. Your completion and return of the attached survey associated with this research project shall be interpreted as your implied consent. Please contact me if you have any questions and/or concerns relating to the above statement and/or the attached survey.


The first 5 questions are general information and the following questions will be done based on a scale that will arrange information on a range from 1 for Strongly Disagree to 5 for Strongly Agree. Please choose answer for each statement. The final 2 questions are completely optional.
