1. Introduction
  Thank you for your time in completing this survey. The information I gather from this survey will go into the thesis project for my Doctor of Ministry degree at McCormick Seminary. In my thesis project, I am looking at characteristics of faith-based poverty abatement and social justice ministries. There are other surveys that are going to the agencies themselves, and to guests who use such services. The conclusions and discussions that I draw from the information gathered via this survey will not include any mention of specific agencies or persons.
This survey assumes that you are the pastor, minister or equivalent for a church, synagogue, mosque, or other congregational entity that has a social service agency/ministry as part of the congregation’s activities. If your congregation hosts such an agency but the agency is independent of your congregation (it’s separately incorporated, and does not answer to your leadership), please do not complete this survey.
There are approximately a dozen questions to this survey; I anticipate that it will take you approximately 20 minutes to complete. If you need to stop in the middle of the survey, you will be able to come back and start from where you left off (or go back to the beginning to change your answers, should you wish to). Once you have completed the survey, however, you will not be able to open it again; the survey can be filled out only once by any individual. Please also note that many of the questions require an answer; you will not be able to move back or forward from a page until all those questions have been answered.
If you are interested in hearing more about this project, and/or you are willing to provide input for later stages of this project, you will have a chance to indicate that at the end of this survey. The contact information you provide will not be linked to your answers.
