  As students, in the University of Kansas Department of Psychology and Research in Education & Department of History and Philosophy of Medicine, we are conducting a research project on The factors involved in spreading HIV among American resident Ethiopians. I would like to ask you a few survey questions to obtain your views on factors that increase the spread of HIV among Ethiopians living in America. As this survey is strictly anonymous your identity will be protected, you are not required to provide any personal information. Furthermore, there will be a locked box where you can place your survey to ensure confidentiality. This survey will take fifteen minutes to complete. You have no obligation to participate and you may discontinue your involvement at any time.
Participation in the survey indicates your willingness to take part in this study and that you are at least 18 years old.
