We are researching the music listening experiences of people with all levels of hearing loss who use Hearing Aids or Bone Anchored Hearing Aids (BAHA).
This survey will ask you how often you listen to music, why you listen to music and about the music you enjoy. You will also be asked about your hearing loss and the hearing aid(s) that you use.
The survey will take about 30 minutes to complete. If you leave your name and email address you will be entered into a prize draw for a chance to win one of three £75 prizes. If you would like to receive information about the results and/or discuss your responses in more detail, you can leave your contact details at the end.
To take part you must
• wear one or two hearing aids for more than 1 hour a day,
• be 18 years or older
If you have a cochlear implant you will not be able to take part.

(If you use the buttons on your web browser you will lose any information you have added to that page)
• You can leave the survey and return to finish it later but you will need to log back in using the same computer.
• A * next to a question means that you need to answer the question before you move on to the next one.
